To complete your criminal record check & vulnerable sector check, please note that you go in person must sign the forms at the RCMP detachment, and bring 2 pieces of valid ID.   Please print off and bring the attached letter from Association with you.

Once your check is completed, return to President.  If you have any questions or need further information, please speak with President.

The Child Abuse Registry check forms are available for print off here on this page as well. Please contact the Pas Minor Hockey Registrar to complete the application process (signature and confirmation of two (2) pieces of ID) and the Registrar will send the application in for review/approval.
