To complete your Criminal Record and Vulnerable Sector Check forms, either print forms (below) or obtain from RCMP detachment office. Once filled out, they must be returned to RCMP office along with attached volunteer letter signed by TPMHA Executive President. 2 pieces of valid ID (one photo ID) are also required to submit to RCMP.

Once completed and retrieved from RCMP detachment office, return to President.  If you have any questions or need further information, please speak with President.

The Child Abuse Registry check forms are available for print off here on this page as well. Please contact the Pas Minor Hockey Registrar to complete the application process (signature and confirmation of two (2) pieces of ID) and the Registrar will send the application in for review/approval.
